| Desmin is a 53 kD cytoplasmic protein, consisting of a nonhelical N-terminal headpiece, a C-terminal tailpiece, and an alpha-helical middle domain of approximately 300 amino acids. Desmin shares 30-70% amino acid homology with other intermediate filaments, but the antibody does not stain other filament proteins such as vimentin, keratin, glial fibrillary acidic protein, and neurofilament protein. Desmin is a marker for myogenic differentiation; it stains smooth muscle cells, myocardium, and striated muscle cells. It does not stain epithelial and myoepithelial cells, endometrial stroma, ovarian stroma, lymphoid tissue, osteocytes, chondrocytes, or mesothelial cells. In neoplastic tissue, desmin stains leiomyoma (12/13 cases), rhabdomyoma (6/6 cases), rhabdomyosarcoma (31/31 cases), and leiomyosarcoma (18/26 cases). Alveolar and embryonal rhabdomyosarcomas consistently stain positive with desmin. In addition to myogenous cells, desmin also stains myofibroblasts. Desmin focal staining may be seen in myofibroblastic differentiated tumors, including fibromatosis and dermatofibrosarcoma protuberans.
